There's a place for the books that everyone seems to recommend and are on all the lists, but let's hear something new. Sometimes I just close my eyes and choose a book from a shelf in the fiction section of my favorite bookstore. If it's something I've never heard of and have never seen on the table displays, and if it sounds like something I'd enjoy, I give it a try. I've found some wonderful books this way. What's an underknown/unappreciated gem that you can recommend?
As a shout out, the most recent one that I loved is "The Axeman's Carnival", about an injured magpie who is rescued by a woman in an abusive relationship. Read it in two days.

Trials of An Expert Witness – by Harold Klawans. Recounts criminal and civil trials where Dr Klawans, an eminent neurologist, testified. House MD meets Matlock; some fascinating cases which explore interesting aspects of neurology, medicine and law (e.g a sleepwalker who kills – was he really asleep and is he culpable); very well written, sometimes tense due to adversarial aspect of the trial, superb insights and some intriguing medical mysteries.
