Adult Dark Fantasy Recs

Hello! 👋
I'm looking for dark fantasy titles recs. That's a wide net to cast but I'd really like anything with one or more of the following:
1. Dual Narrations
2. Anything to do with (old) Gods
3. Magical hierarchies/power imbalances
4. Medical themes (especially if it involves a mystery around sickness but I don't want something apocalyptic. dystopia is fine)
5. Complexities around immortality or long life spans. Not too interested in supernatural elements like vampires etc. More so magic related stuff.

I'm looking for something with a good plot where any romance is more of a side plot. Bonus if that romance is LGBTQ+. Also totally okay with heavy themes.
