I read a lot of horror, and the only things that I have ever read and made be feel icky and disturbed are as follows:
Let the Right one In – John lindvist
Lapvona – Ottessa Moshfegh
Most of Cormac Mccarthy, maybe try the road, but outer dark was much more bleak and disturbing to me
Earthlings – Sayaka Murata
The troop – nick cutter
The southern bookclubs guide to slaying vampires – Grady hendrix gave me huge ick because of the age of the victims and how the author describes it. I hated that book but lots of folks loved it. I do typically love Grady hendrix but that was disturbed me in a way that was too real. I really loved How to Sell a Haunted House.
The Ruins by Scott Smith seems to be one that freaks people out but I hated it. Couldn’t stand any of the characters
Those are the ones that I can remember off the top of my head.

I hate that Barnes & Noble never ever ever ever ever has any of these titles in stock.

Manhunt by Gretchen Felker-Martin. References the older story [The Screwfly Solution](https://pseudopod.org/2014/08/22/pseudopod-400-the-screwfly-solution/) by James Tiptree Jr.
Sister, Maiden, Monster by Lucy A. Snyder. One of the most horrifying books I’ve read.
The Girl Next Door by Jack Ketchum, it’s very disturbing and it’s based a true story.
*The Haar* by David Sodergren. Very gruesome.
You might try books by Preston Child, two writers who use their combined last names.
Grady Hendrix novels can be pretty graphic imo

There is actually a genre called “extreme horror” so you may want to search specifically within it. [Here](https://www.reddit.com/r/ExtremeHorrorLit/comments/177q70d/what_is_your_absolute_favorite_extreme_horror/) is a post from a few months ago in /r/extremehorrorlit that might be a good start.
