Maybe the main character has a desire in life but can't fulfill it because it hurts to put in the effort to get it.
Maybe the main character is trapped in a bad situation in life but can't get out because it hurts to come to terms with it and it hurts to take the step away from it.

Probably *My Year of Rest and Relaxation* but she’s not necessarily like relatable or creating sympathy.
