I mean people who just feel little to no empathy less than others and how this one factor in their brain affects their lives and/or the people around them. Be it those who chose to learn how to be empathetic or those who are just "meh" and goes on with their lives regardless of how they come across to others.
They can be the MC, the side characters, and even the antagonists. Just not in a way that they're portrayed as automatically evil and/or sadistic monsters just because of something that's not within their control.
