This doesn’t fully fit what you are asking for but I’d recommend The Spirit Bares its Teeth by Andrew Joseph White. It’s a fantasy book with some horror elements. The mc is a trans man and the love interest is a trans woman. I’d also recommend The Lilac People by Milo Todd.
